Up to 1 in 5 pregnant and postpartum people will experience a perinatal mental health disorder. Up to 1 in 10 partners will too. And 75% of them will not get treatment. It is the leading cause of pregnancy related death in the United States.


As a doula, you are often the most consistent presence in your client's perinatal year. You see things their provider does not. You hear comments their family brushes off. You notice when something shifts.

  • Why perinatal mental health is bigger than postpartum depression, and why bipolar disorder is the diagnosis missed most often
  • The risk factors that stack and raise a client's risk significantly, including the systemic ones clinical training often skips
  • Why doula support itself is a protective factor, with research to back it up
  • How to use the EPDS-US (the updated, trauma informed version of the Edinburgh Postnatal Depression Scale), plus the PHQ-2, PHQ-9, and GAD-7
  • The exact language to offer screening at a first prenatal visit, in postpartum, and when something feels off
  • Red flags that require same day action and a crisis pathway you can follow
  • How to make a referral your client will actually act on, including the two-name approach and the warm handoff
  • How to document screening without diagnostic language
  • What to do when a client declines support, when confidentiality has to be broken, and how state law affects your reporting obligations
  • How to take care of yourself so this work stays sustainable
